Which document shall be mandatory?

The choice of mandatory documents shall follow the principle of which processes and process outputs are most critical to the project management process.
This is not the matter of the document name, title or template, (some of which might be standardized by your organization) but the information required shall be the measure of choice.

Examples:

During Conception: 

• Project Charter

During Initiation: 

• Project Handbook (based on Project Charter )

During Execution/Work

• Phase Plan (part of Project Handbook ) 
• Performance Reports (part of Project Handbook ) 
• Lessons Learned Report (part of Project Handbook )

During Closure 

• Client Sign-Off (based on Project Handbook )
• Internal Sign-Off (based on Project Handbook )

If mandatory document is to be omitted, it must be properly documented (motivation) in the Project Handbook .
This is in the responsibility of Project Manager to decide which further documents will be used (and then mandatory) to the particular project – subject to be documented within the Project Handbook .
